This isn't academic language—it's the requirement that trips up most organizations during audit.

The standard requires you to:

  • Define and apply risk assessment criteria, including risk acceptance criteria and criteria for performing assessments
  • Ensure repeated assessments produce consistent, valid and comparable results
  • Identify risks associated with loss of confidentiality, integrity, and availability for information within the ISMS scope
  • Identify risk owners for each identified risk
  • Analyze and evaluate those risks against established criteria

That consistency requirement is where I see 80% of failures during certification audits. If five people in your organization assess the same scenario, they should reach similar conclusions. Most don't—their methodologies are too subjective or poorly defined.

The standard deliberately avoids mandating specific methodologies. You can use quantitative approaches (financial modeling, statistical analysis), qualitative methods (descriptive scales), or hybrid combinations. But whatever you choose must be documented, consistently applied, and defensible under audit scrutiny. This flexibility becomes a trap when organizations interpret it as "anything goes."

Step 1: Establish Risk Criteria That Actually Work

This foundational step gets rushed constantly, yet it determines everything downstream. Your risk criteria define the scales and thresholds that make your assessments meaningful. Without properly calibrated criteria, your entire risk register becomes an exercise in wishful thinking.

I audited a financial services firm that rated a potential £10 million data breach as "medium risk" because their impact scale topped out at "significant financial loss"—which they'd defined as "more than £100,000." For an organization processing billions annually, this scale was meaningless. Every major threat looked like medium risk because their criteria couldn't express the true scale of potential impact.

Building Impact Scales That Reflect Reality

Don't copy generic templates. Interview your executives. Ask: "At what financial threshold would you expect to be called immediately?" That's probably your critical impact level. Work backward to establish meaningful gradations.

Document how you'll handle scenarios where a risk scores differently across dimensions. Do you take the highest score? Average them? Use weighted calculations? The approach matters less than consistency and documentation.

Likelihood Scales That Mean Something

Avoid vague terms like "unlikely" or "possible." Define likelihood in terms your organization can actually assess:

  • Quantitative approach: Annual probability percentages or expected frequency
  • Qualitative approach: Historical occurrence patterns ("occurred multiple times in similar organizations," "occurred once in our industry in past five years")
  • Hybrid approach: Ranges that combine historical data with expert judgment

The key is ensuring different people can look at the same threat scenario and reach similar likelihood assessments using your defined criteria.

Step 2: Asset Identification Drives Everything Else

You can't assess risks to assets you haven't identified. Clause 8.1.1 requires asset inventories, but most organizations approach this mechanistically—spreadsheets of hardware and software that miss the information assets that actually matter.

Focus on information assets first, then work backward to supporting systems and processes. Consider:

  • Primary information assets: Customer data, intellectual property, financial records, strategic plans
  • Supporting systems: Applications that process primary assets, infrastructure that hosts applications
  • Supporting processes: Business processes that handle information assets, security controls that protect them

For each information asset, document its confidentiality, integrity, and availability requirements. A customer database might require high confidentiality (personal data protection), high integrity (accurate records), and medium availability (business hours access acceptable). These requirements drive your risk assessment approach for that asset.

What Auditors Look For in Asset Inventories

During audits, I examine whether organizations have identified assets that align with their business processes and information flows. I look for:

  • Completeness: Do major business processes have corresponding information assets?
  • Classification: Are confidentiality, integrity, and availability requirements documented?

Threats only matter when they can exploit vulnerabilities. For each identified threat, document the vulnerabilities it could exploit:

  • Technical vulnerabilities: Unpatched systems, weak configurations, inadequate access controls
  • Physical vulnerabilities: Unsecured facilities, inadequate environmental controls
  • Administrative vulnerabilities: Inadequate policies, insufficient training, weak processes

Cross-reference this with your Control 12.6 (Management of technical vulnerabilities) implementation. Organizations with mature vulnerability management programs can leverage existing vulnerability data in their risk assessments.

Step 4: Risk Analysis That Produces Defensible Results

Now comes the actual risk analysis—combining threat likelihood, vulnerability exploitability, and potential impact to produce meaningful risk ratings. Your methodology must be repeatable and produce comparable results across different assessors.

Qualitative Risk Analysis Approach

Most organizations use qualitative methods because they're more intuitive and require less precise data. Structure your analysis around standard risk formulas:

Tip: Use the simple formula: Risk = Threat × Vulnerability × Impact. Rate each component on your established scales (e.g., 1-5), then combine them using multiplication or lookup tables. Document your combination method clearly.

For each risk scenario, document:

  • Threat assessment: How likely is this threat to occur in your environment?
  • Vulnerability assessment: How exploitable are the relevant vulnerabilities?
  • Impact assessment: What would be the consequences if this threat successfully exploited vulnerabilities?
  • Overall risk rating: Combined assessment using your documented methodology

Ensuring Consistency Across Assessors

The "consistent, valid and comparable results" requirement means different people should reach similar conclusions when assessing the same scenarios. Achieve this through:

  • Standardized worksheets: Templates that guide assessors through your methodology
  • Reference examples: Sample scenarios with completed assessments that demonstrate your approach
  • Calibration sessions: Regular exercises where multiple assessors evaluate the same scenarios and compare results
  • Review processes: Senior review of risk assessments before finalization

Step 5: Risk Evaluation and Treatment Planning

Clause 6.1.3 requires risk treatment planning for risks that exceed your acceptance criteria. This isn't about eliminating all risks—it's about making informed decisions on which risks to treat and how.

For each risk that exceeds your acceptance threshold, consider the standard treatment options:

  • Risk modification: Implement controls to reduce likelihood or impact (most common)
  • Risk sharing: Transfer risk through insurance or contracts
  • Risk avoidance: Eliminate activities that create unacceptable risks
  • Risk retention: Accept the risk with full knowledge of consequences

Your risk treatment decisions should align with your Annex A controls selection. If you identify a risk related to access control weaknesses, your treatment plan should reference specific controls like Control 9.1 (Access control policy) or Control 9.2 (Access to networks and network services).

What Auditors Examine in Risk Treatment

During audits, I verify that risk treatment plans are actually implemented and effective. I look for:

  • Evidence that treatments are actually reducing risk levels

Don't copy someone else's risk register. I've seen identical risk registers across completely different industries—a clear sign they started with a template and never customized it. Your risks should reflect your specific assets, threats, and business context.

The Annual Exercise Syndrome

Clause 9.3 (Management review) requires regular risk assessment review, but many organizations treat this as an annual paperwork exercise. Effective risk assessment is ongoing—major changes to systems, processes, or threat landscape should trigger risk reassessment.

The Metrics Obsession

Some organizations get so focused on perfecting their risk calculation formulas that they lose sight of the goal: making better security decisions. Your methodology should be "good enough" to produce consistent, defensible results—not mathematically perfect.

Measuring Risk Assessment Effectiveness

How do you know if your risk assessment actually works? Look for these indicators:

  • Decision utility: Does leadership use risk assessment results to make security investment decisions?
  • Control alignment: Do your implemented controls address your highest-rated risks?
  • Incident correlation: When security incidents occur, were the related risks identified in your assessment?
  • Consistency metrics: When multiple assessors evaluate the same scenarios, how closely do their ratings align?

Consider integrating your risk assessment with related ISO 27001 processes. Risk assessment results should inform your internal audit program priorities and feed into your management review discussions.

For organizations operating in specific sectors, consider sector-specific guidance. ISO/IEC 27017 provides additional controls for cloud services, while ISO/IEC 27018 addresses privacy protection in cloud environments. These standards can inform your risk identification process if you operate in these areas.

Risk assessment isn't a theoretical exercise—it's the foundation that makes your entire ISMS meaningful. Get it right, and you have a living tool that actually improves your security posture. Get it wrong, and you're building an expensive monument to compliance theater.
